| Flicka Registration/Volunteer Agreement 01-02
By signing the following registration and volunteer agreement form, you agree to fulfill your volunteer hour commitment to Flicka:
You also agree to submit:
1.) 10 post-dated cheques for training fees
2.) one cheque post-dated to June 30th, 2006 which is your Volunteer Commitment cheque. This cheque will be returned to you on
June 30th, 2002 when your volunteer hours have been fulfilled.
PLEASE NOTE THIS REGISTRATION/VOLUNTEER AGREEMENT PACKAGE MUST BE SIGNED BEFORE YOUR GYMNAST STARTS TRAINING IN SEPTEMBER 2005.
